Families

How do I access services for my family? How can CFC help me and my family?

Your family can access help from Coordinated Family Care if you are the parent/guardian of a youth between the ages of 5and 21 years old with social, emotional, and behavioral needs by contacting Value Options (the Contracted Systems Administrator) at 1-877-NJ-CSOCI Or 1-877-652-7624. Value Options is the referral gatekeeper for The New Jersey Division of Children’s Behavioral Health Services. They can help you access appropriate services for you and your family.

What are the eligibility requirements to receive services by CFC?

To receive services from CFC for you and your family you must first:

  • Be the parent/guardian/caregiver of a youth between the ages of 5 and 21 with a social, emotional, or behavioral complex need.
  • Provide written and verbal parental consent for the youth under 18 to be referred. Youth over 18 years of age can make their own referrals and receive services as long as they at one point in time received services before they turned 18.
  • Reside in Middlesex County (Value Options will get you to the right agency if not a MC resident)
  • As of yet there are no financial requirements to receive services from CFC.

Explain to the Care Coordinator your family situation and any and all services you may be receiving at the present time or in the past.

If CMO is the appropriate level of care for you and your family you must agree on a voluntary basis to participate in meetings, create a crisis plan, complete some paperwork and have weekly contact with your assigned Care Manager.

Will my care be discussed with any one else?

CFC requires a release of information once your referral has been received by Value Options to discuss any matter regarding your family. All information is kept in the strict confidence however CFC is a mandated reporter and we are required by law to report any suspicion of abuse or neglect to DYFS.

How can I become a provider for Coordinated Family Care and the NJ Children’s System of Care?

What types of services do youth/families utilize or need once enrolled in the CMO?

Youth/Families enrolled in CFC utilize a variety of both traditional and non-traditional services. CFC is always looking to partner with local agencies who share in our mission of keeping youth with their families and in their communities. Some of the utilized services include but are not limited to the following: Summer camps, outpatient mental health services, after school programs, mentoring services, crisis stabilization programs, arts and crafts instructional courses. If you would like additional information about the services needed please contact the Resource Development Department.

What is the process in becoming a provider for CFC or the NJ Children’s System of Care?

To become a service provider for CFC a face to face meeting with our agency must occur. This meeting will allow us to discuss the needs of youth/families and how your service may help us meet these needs. If a partnership is formed, CFC will mail you an Agreement and a Business Associate Agreement which outlines the documentation and credentials required to be service provider. Once a fully completed packet is recieved and approved we will move forward and set up a presentation of your service to our staff – which will then generate referrals on an as needed basis.
Initiation of an Agreement does not guarantee active status and/or youth/family need for services. Service requests determines referrals.

Community Members

How can Community Representatives become involved with CFC youth/families?

Coordinated Family Care is eager to partner with community agencies/representatives for several reasons. We rely on community supports for:

  • Volunteer opportunities for our youth/families
  • Free and sustainable resources
  • Financial assistance for youth/families
  • When planning or coordinating a youth/family event (especially during the holiday season)

We also want to hear from you as to how we can include our youth/families in events that you may be sponsoring or planning. Some of the community representatives we have partnered with in the past include the Perth Amboy Police Department, United Way of Central Jersey, Health Net Health Plus, Embassy Suites and The New Brunswick Police Department.
